BMI = (weight in pounds / height in inches squared) x 703

(154 ÷ 4,761) × 703 = 22.7

Where BMI Falls Short

BMI has one task and one job only. Compare weight to height. It was never designed to assess body fat, and it can’t distinguish between fat and muscle.

  • Where the fat is on your body (fat around your waist is a distinct risk than fat elsewhere, and BMI can’t see it)
  • Bone density
  • Body composition and aging
  • Differences in health risk between ethnic groups at the same BMI number

Age counts, too. As people age, they lose muscle and acquire fat even if the scale does not change much, so the same BMI figure at 30 and at 60 can mean very different bodies. And again and again studies have demonstrated that health risk at a certain BMI is not the same among ethnic groups. For example, certain national health agencies now propose lower BMI cut-offs for certain Asian populations since health hazards such as type 2 diabetes tend to develop at lower BMI levels than what the conventional adult ranges would suggest .

This doesn’t mean BMI is meaningless. It is cheap, fast and quite effective in large groups and that’s exactly why it’s still the first figure most doctors look at. The problem comes when it is taken as a full answer to one single person, not a starting point.

Part 2: Lean Body Mass Calculator

Lean body mass (LBM) is everything in your body that isn’t fat. Muscle, bone, organs, and water. Then take your fat mass and subtract it from your total weight. What’s left is your lean mass.

The Boer formula is the most often used formula and is separated by sex, as men and women have differing typical ratios of muscle, bone, and water.

Boer formula, male:

LBM = (0.407 x weight in kg) + 0.267 * height in cm – 19.2

Worked example: 180cm, 80kg.

(0.407×80) + (0.267×180) – 19.2 = 32.56 + 48.06 – 19.2 = 61.4 kg

Boer formula, female:

LBM = (0.252 × weight in kg) + (0.473 × height in cm) – 48.3

Worked example: 165 cm 65 kg

(0.252 × 65) + (0.473 × 165) – 48.3 = 16.38 + 78.05 – 48.3 = 46.1 kgs

You will also encounter the James formula, which is employed in some therapeutic situations. It takes comparable inputs but weighs them slightly differently. Results from the two formulas are generally near but not identical which is expected for an estimate rather than a direct measurement.

What Lean Body Mass Is Used For
